Online registration

Each online registering participant will receive a PRN - Participant Registration Number at the end of registration procedure to secure participant's identification. Please make your print out of your Registration Review where you will find your PRN!
 
All participants and accompanying persons are kindly requested to register in advance by completing and submitting the Registration Form with the appropriate fees. All participants registering after April 15, 2008 are kindly requested to make all payments by credit card exclusively or to register on-site.

Registration Fee

Registration fee includes:

• Access to meeting spaces and exhibition
• Congress bag including the one copy of Final programme and Abstract book
• Coffee breaks
• Opening ceremony, Welcome reception at the Clarion Congress Hotel Prague

Colposcopy Course

Available for registered participant only. Please be informed that the course will be held if the number of participants is more than 50.

Students

Participants applying for student fees should fax a proof of their student status ( a photocopy of the university identity card) to CZECH-IN, Attn: HPV 2008, Fax: +420 261 174 307

Confirmation

Upon receipt of the correct registration fee, each participant will receive a confirmation of registration. Please bring this confirmation to the registration desk at the Clarion Congress Hotel as proof of your registration. Confirmation will be sent upon receipt of payment.

Payment

All fees for the Congress registration should be paid in EURO (EUR), free of all bank charges to the CZECH- IN – clearly stating the participant‘s name and PRN (Participants Registration Number)

Responsibility

The participant acknowledges that he/she has no right to lodge damage claims against the organisers should the holding of the Congress be hindered or prevented by
unexpected political or economic events or generally by force majeure, or should the non-appearance of speakers or other reasons necessitate programme changes. With registration, the participant accepts this proviso.

Cancellation / Refund Policy

Refund of registration fees, less 30% administrative charges, can be applied for in writing until April 1,  2008 to the Organising Secretariat in Prague / Czech Republic. After this date no refund will be possible. Registrants may transfer their registration to a colleague at any time. For any change of name a fee of EUR 30.– will be charged.
